This course is designed to focus on healthcare and social care provision to individuals, groups and communities within specialist community services.

The emphasis in practice is on developing your competence and confidence in assessing the needs of individuals, groups and communities, and working in partnership with them to make informed health choices.

The course will equip you with the skills, knowledge and value base to enable you to function within the community and become an autonomous, reflective, evidence-based and innovative practitioner, as well as help you to deliver an effective, clientcentred and holistic programme of care.
